I am 16 and got a server job at Steak and Shake in Ky. The manager told me to go to orientation. On the paper, it said they require "wage and age agreement" form and "youth at work initiative" form. However, the manager didnt say anything about these forms. I cannot find them online either. Help!! Are these forms necessary? Where to find them?!!
